Everything you need to know about making a rehabilitation, medical and other health service referrals to Brunswick Private Hospital.

Brunswick Private Hospital rehabilitation works in partnership with physicians and general practitioners to provide the best possible environment for your patients requiring inpatient care for their rehabilitation. 

To refer, please follow our simple five-step process.

  1. Does your patient need a rehabilitation admission to hospital (inpatient or outpatient)?
  2. Does your patient have Private Health Insurance, DVA Status, Work Cover or the ability to self-fund their admission?

  4. Contact our admissions team on (03) 9385 1189 if you require any further information or assistance.
  5. Our admissions team will arrange an assessment and admissions plan.

Inclusion Criteria

  • Medically stable, do not require acute care/higher level care and are 17 years of age and over.
  • Patients requiring rehabilitation care following an injury or illness with the express purpose of improving functioning and discharging home.
  • Patients with multidimensional needs associated with medical conditions related to ageing have treatment goals in improving their functioning.
  • Patients who require review, treatment, and management by a rehabilitation, geriatrician, or general medicine physician and multi-disciplinary team.
  • Patients who can demonstrate, consent, and agree to participate in daily Rehabilitation therapies.
  • Patients who have a capacity to learn and process new information.
  • Patients who have a defined discharge destination.
  • Patients with depression where depression is a comorbidity not the principal diagnosis.
  • Bariatric patients <150kg who will need detailed assessment prior to admission approval.
  • Patients who are confirmed PCR negative for COVID-19 within a maximum of 48hrs prior to transfer.

Exclusion Criteria

  • Patients who are medically unstable, require acute/higher level care or have complex social needs.
  • Patients who require non mobile equipment/fixed lifting equipment/hoists.
  • Patients who are cognitively impaired or suffering with delirium i.e., Risk of wandering.
  • Patients who had an admission diagnosis of alcohol or drug intoxication or psychiatric disease.
  • Patients requiring peritoneal/hemodialysis.
  • Patients with a tracheostomy insitu.
  • Confirmed COVID positive patients or COVID suspected patients.
  • Patients where primary reason for admission is palliative care.

Brunswick Private Hospital Rehabilitation Assessors and its Management team reserve the right to decline any referrals where we may be at risk of not providing the level of care or outcome that is required by the patient.
